There are two versions of each form:

(1) Fillable PDF Form: The fillable PDF form works the same as any other online forms. It allows you to enter information, select from the drop box and also check fields as needed. You can complete the form online or save a copy of the form on your computer. The forms don't have the ability to submit online so you must then print the completed form, sign it and submit via regular mail.

(2) Original PDF Form: This is a blank form for printing only. You will not be able to pretype any information.

IRRB Request for Proprietary NLU Data

This is the form used when faculty and staff want to use data belonging to the university for purposes of institutional assessment or personal research. Guidelines are included.

IRRB Change of Status Request Form

Any change to a previously submitted protocol, such as, extension of time, the addition of investigators, a change of study method, or a change of location, requires the submission of a change of status form along with appropriate documentation. If a new protocol is required, the primary research investigator will be notified.
