First Time Educational Benefits Users
The following is required before requests for Veterans Benefits can be processed:
- Be admitted to National-Louis University
- Complete an Application for VA Educational Benefits (
Form 22-1990)
[If Chapter 35, use
Form 22-5490.] - Return the form along with a copy of Form DD-214 to:
Attn: VA Certifying Official
c/o Office of the Registrar

If choosing to submit the necessary documentation directly to Department of Veterans Affairs, please forward copies to the above address. The National-Louis University VA Certifying Official will complete Enrollment Certification (Form 22-1999) and send it to the Department of Veterans Affairs at the end of the add/drop registration period for each term of enrollment.
Educational Benefits are paid directly to the student, and the student is responsible for tuition payments to the University. Chapter 31 Vocational Rehabilitation benefits are paid directly to National-Louis University.
Educational Benefits Users (from another institution)
The following is required before requests for Veterans Benefits can be processed:
- Be admitted to National-Louis University
- Complete a Request for Change of Program or Place of Training (
Form 22-1995)
