The following tool is administered by Cooperating Superintendents only. Cooperating Superintendents must evaluate interns at the mid-point and end of their internship. After completing the mid-point and final evaluation, please click on the "Submit Form" button. AFTER you click on "Submit Form" on the evaluation, please click on "CREATE A PRINTABLE VERSION" (located at top on the right) to print a copy of your completed evaluation. Please print a copy for later for when you meet with the intern and his/her NLU advising professor. It is also possible to copy the URL of the "printable version" and save it for later. And a third method for saving it is to go to Save As in your browser's menu. After you have printed and/or saved the "printable version" of your submitted competency appraisal, then send an email to the intern to let him/her know that you have completed the evaluation. This email will serve as a confirmation to the Advising Professor that the mid-point and final evaluation was completed by the Cooperating Superintendent. This also is a signal that a review should be scheduled for all three to meet. At no point is the student expected to place the CA into their portfolio and the submission is best presented to the student during the review meeting.

The EDL Doctoral Program Competency Appraisal takes about 5 minutes to complete. Your responses cannot be saved until after you submit the entire form. Therefore, the appraisal should be completed and submitted in one sitting.
***Please note: If you are unable to find the printed or saved Competency Appraisal, there are two options: 1.) Fill out the Appraisal again and notify email@example.com or firstname.lastname@example.org ,and 2.) request the data be sent to you from email@example.com. The data that will be sent will be in spreadsheet form and look much different that the "printable version" that the superintendent has submitted.
