Follow the guidelines below before submitting request form:
- The NLU video/DVD collection is for educational purposes.
- Only NLU faculty may check out videos and DVDs. To request a video or DVD for classroom use, please fill out the request form below.
- NLU students may request and view videos and DVDs in the Library only. Students who need a video or DVD for classroom use must ask the instructor to check out it for them.
- Videos and DVDs may be checked out to faculty for two weeks. To renew videos or DVDs, call the Wheeling Campus Library at 1-800-443-5522, x5515 or x5335.
- Up to 5 videos may be ordered on one form. If you need to order more than 5 videos/DVDs, additional forms may be submitted.
- Forms may be submitted in print or electronic format.
- If using the online form, please specify telephone number, college, department and e-mail, so that our staff can contact you if there is any question.
- Please complete each form by filling in all boxes. Incomplete forms may delay the processing of your request.
