Degree Finalization Form
In order to be awarded a degree, you must submit the form below by the appropriate deadline. Your transcript and diploma will reflect the degree date that you select on the second page of the form. After you submit the form, we will initiate the degree finalization process, which entails verifying that all of your degree requirements have been met, posting your degree to your transcript and issuing your diploma. Degree finalization must occur regardless of whether you plan to participate in the commencement ceremony.
After requesting degree finalization, you are eligible to participate in the commencement ceremony in your regional area. You will have the option to register for commencement on the second page of this form.
If you find that you will not complete your degree requirements by the date for which you have applied, you may request a change in the degree award date. Simply resubmit this form with the new date selected.
Confirmation of your degree finalization request is sent to your student email address.
