Requests for Name and Address Changes
Changes to the personal information of an academic record should be requested in writing (on paper or electronically) and directed to the Office of Admissions and Records (OAR). Verification of a legal name change must be provided before academic records can be updated. Documents accepted as proof of a name change include birth certificates, marriage licenses, divorce decrees authorizing the name change, judicial decrees specifically authorizing the name change and naturalization papers. Current and former employees of National Louis University should submit requests for a change of personal information to Human Resources at the Wheeling campus.
Ways to Send Your Request
You may send change of name or address information to us by mail, fax, e-mail or in person:
- By mail: Send the completed form and copies of any necessary documentation to our office address
- By fax: Send the completed form and copies of any necessary documentation to the Records office fax number
- By e-mail: Current students may now request address changes by sending an e-mail with the changes (address only) directly from their Portal accounts. Visit MY.NL.EDU to log in.
- In person: Bring the completed form and any necessary documentation to OAR on the first floor of the Wheeling campus from 9am-5pm, Monday through Friday. You may also bring the completed form and copies of any necessary documentation to a Student Service Center at any National Louis campus.
Online Change of Address
If you are a current student, you may now make address changes by logging into your portal account, selecting the "My Services" tab in the portal, selecting the sentence link mid-page and then choosing "Personal Information." You may not use this feature if you have ever been an employee of the University. Go to MY.NL.EDU to log in.
