How to apply for Type 73 Certification

When you finish the requirements for Type 73 certification, send the following documents to Dr. Marie Bracki on the Lisle campus (address below).

1.  Copy of Basic Skills Test pass sheet (only for non-teachers).

2.  Copy of the Type 73 Test pass sheet.

3.  Copy of their Curriculum Plan(s).

4.  Copy of unofficial transcript.

To begin the paperwork process, you should call Barb Wilkovich (847-465-0575 ) in the certification office in Wheeling. The Type 73 form is then sent to me for Dr. Bracki for her signature. She cannot sign forms if she does not have all of the previously listed materials.
