National-Louis University Health Services

The university does not have a health professional on campus available to students. The Office of Student Affairs is committed to adhering to the regulations of the Illinois Department of Public Health and other regulatory agencies. All medical records and services are confidential. The University does not provide student health insurance, nor does it make recommendations for obtaining student health insurance. Students with questions or concerns should contact the Office of Student Affairs.

Immunizations

The Illinois Department of Public Health has finalized the immunization requirements for all postsecondary educational institutions.

Students born before January 1, 1957, may be required to submit a medical history and information form, or to provide University Health Services with proof of birth (i.e., birth certificate, driver’s License, or personal identification card issued by the Secretary of State) for exemption. Student born on after January 1, 1957, may be required to submit to Student Affairs proof of immunization to measles, mumps, rubella, tetanus, and diphtheria.

We do provide copies of your medical records to you. Records can be released to the student after he or she signs a medical release authorization form. Please contact the Office of Student Affairs for the medical release authorization form.
