Students who receive federal Title IV funds (from Stafford Loans, Perkins Loans, Pell Grants, SEOG) are required to complete a Title IV Authorization Form. This form allows you to authorize/not authorize NLU to apply these funds to pay cost of attendance charges (other than tuition and fees and room and board if contracted for room and board) including unpaid charges remaining from previous terms within the current academic year. This is a one-time requirement and is valid for the entire period of enrollment. Please print the form and submit it to the Student Finance Office.
