Permissions Request Form

To request permission to reprint copyright protected material, complete this form and submit it along with, (1) a photocopy of the material you wish to use, (2) a photocopy of the copyright page, and (3) a photocopy of the front cover of the original source. Upon receipt, the request will be processed and then forwarded to the appropriate copyright owner for their approval. Please note that the process may take 6-8 weeks (sometimes longer) to complete, so plan ahead.
